Places for People uses data, our emerging expertise with primary care medicine, and our understanding of the needs and challenges for people living with serious and persistent mental illness, substance use disorders, physical health needs and poverty, to support the individuals we serve. Our Integrated approach will result in improved health outcomes and greater health literacy for our staff and the individuals and families we serve.
Mental and physical health are connected
Studies indicate that people with serious mental illnesses die an average of 25 years earlier than the general population. However, the causes of death are medical illnesses that were not treated or risk factors that could have been modified, including high blood pressure, diabetes, or substance use.
Places for People is committed to improving the health of the people served. The majority of persons served by PfP have at least one chronic physical health condition in addition to a serious mental illness or substance use disorder, and most also have significant histories of trauma that has been unidentified and, therefore, untreated.
Places for People is using our data, our emerging expertise with primary care medicine, and our understanding of the needs and challenges for people with serious and persistent mental illness to craft approaches that will result improved health outcomes and greater health literacy for our staff and our clients.
